Insurance Participation

patient-infoWe participate in traditional Medicare and many private medical insurance plans.  Major insurance companies include:  Anthem, Aetna, CIGNA, Commercial United Health Care, Humana, Medical Mutual, Ohio Medicaid, as well as many smaller plans.  There are a few vision plans in which a select group of Apex Eye doctors also participate.  With each insurance company offering many different plans and with plans constantly changing, it is the patient’s responsibility to understand if Apex Eye is a participating provider for your plan. We do want your entire experience to be positive so each Apex Eye office has staff that can answer questions regarding physician’s participation in specific insurance plans.  You are also need to be aware of specifics regarding deductibles, co-payments, covered services and the need for referrals prior to appointments.

Payment Expectations

For patients with insurance coverage, we do ask that you provide us with complete and accurate insurance information when you schedule your appointment.  This allows us to inform you if we are participating with your insurance.  As a courtesy to our patients, we will process and file insurance claims for all insurance plans in which we participate.  Once the insurance company processes your claim, you may receive an Explanation of Benefits (EOB) from your insurance company that summarizes the charges for services you received, the amounts covered by your insurance, and any remaining balance for which you are responsible to pay.   Depending on your plan, insurance can pay the entire claim, pay part of the claim, or deny the claim.  After Apex Eye receives the insurance payment, you will then be billed for any remaining balance that was not collected at the time of service.  We appreciate your prompt attention to these balances.

If Apex Eye does not participate in your insurance plan, you can still receive services from us as a self-pay patient.  Depending on the potential services desired, you may receive an estimate before the appointment.  For your convenience, we accept MasterCard, Visa, Discover, American Express and HAS/HRA cards. We also accept debit cards, cash and checks.

Note:  Insurance plans use our corporate name, Eye Care Associates of Greater Cincinnati, Inc., rather than Apex Eye.
